The verdict

Apple Watch Series 12 is a full smartwatch, from $399, with a sleep score, sleep apnea and hypertension notifications and an ECG app. It's for anyone with an iPhone 11 or later who wants those features, and no subscription is listed for them. The catch is battery life: Apple rates it at up to 24 hours of normal use, so you'll charge it most days.

The trade-off

You'll charge it most days (Apple rates it at up to 24 hours), Android phones aren't supported, and the notifications flag possible signs rather than diagnose.

About this price

$399 is Apple's lowest Series 12 price and the price it states for the 42mm model. Apple's buy page, including the configuration page for the 42mm Space Gray aluminum GPS case with a Sport Band, shows 'From $399' for the Series 12 lineup. Its lease footnote reads 'For an Apple Watch Series 12 42mm with a purchase price of $399', and the Series 12 page carries the same example. Neither page printed a separate price for this exact band and color, so confirm it at checkout. No sale or strike-through price was shown. The price is before tax and shipping. It doesn't include trade-in credit (the buy page advertises $30 to $430 off with an Apple Watch trade-in), leasing, Apple Card Monthly Installments or AppleCare.

A GOOD FIT FOR

An iPhone owner who wants sleep scoring, sleep apnea and hypertension notifications and an ECG app in one everyday smartwatch, with no required fee.

  • You have an iPhone 11 or later on iOS 27 or later and want sleep scores and heart notifications in an everyday smartwatch
  • You're 18 or older, have no sleep apnea diagnosis and want notifications about possible signs of moderate to severe sleep apnea
  • You're 22 or older, not pregnant and not diagnosed with hypertension, and want notifications about possible hypertension
  • You want health features without a required monthly fee

THINK TWICE IF

  • Anyone with an Android phone, since it needs an iPhone 11 or later
  • Anyone who doesn't want to charge most days (Apple rates normal use at up to 24 hours)
  • Anyone under 22, pregnant or already diagnosed with hypertension, for whom Apple says the hypertension notifications aren't intended
  • Anyone who wants a medical measurement of blood oxygen or temperature: Apple labels both as not for medical use
  • Scuba divers and anyone doing high-velocity water sports such as waterskiing, since Apple rates it for shallow-water activities
What still needs checking
  • Coverage for battery capacity loss beyond Apple's one-year warranty, which excludes batteries that diminish over time unless failure is due to a defect
  • The checkout price of the exact 42mm aluminum GPS Sport Band configuration: Apple's buy page shows 'From $399' and a $399 lease example for the 42mm model, but no price for this band and color
  • What the Sport Band is made of, and the wrist range of the single band size in the box
  • FDA clearance details for the sleep apnea notifications, which weren't retrieved for this review (Apple's Series 12 page mentions FDA-cleared features without listing them)
  • Independent, peer-reviewed accuracy data for the Series 12 sensors (Apple's heart-rate accuracy claim rests on its own study)
  • Battery lifespan, battery service cost and charger power draw

The practical details.

Water and dustWater resistant to 50 m under ISO 22810:2010. Apple rates it for shallow-water activities such as swimming in a pool or ocean, or snorkeling to 6 m. It isn't for scuba diving, waterskiing or other high-velocity water sports. Apple says water resistance can diminish over time. Dust resistance is rated IP6X.

InstallationNo installation

No installation. Pair it with an iPhone 11 or later, including iPhone SE (2nd generation or later), running iOS 27 or later. It doesn't work with Android phones. In the box are the watch, the band and an Apple Watch Magnetic Fast Charger to USB-C Cable (1 m). No power adapter is listed.

WarrantyThe Apple One (1) Year Limited Warranty runs one year…

The Apple One (1) Year Limited Warranty runs one year from the date of original retail purchase. It excludes damage caused by accident, abuse, misuse or other external causes. It also excludes consumable parts, such as batteries, that diminish over time by design, unless the failure is due to a defect in materials or workmanship. The warranty page doesn't give a battery-capacity threshold for service. Apple says AppleCare plans are separate from and in addition to this warranty.

ReturnsUnder Apple's U.S

Under Apple's U.S. Sales and Refund Policy, you can return or exchange it within 14 days of the date you receive it from Apple. It must be in its original condition, with all included parts, accessories and packaging, and you need a receipt. Apple may inspect returned products. The Series 12 buy page says returns are subject to Apple's Sales and Refunds Policy. The pages checked don't state restocking fees or return shipping costs.

More about this assessment

Apple Watch Series 12 is a full iPhone smartwatch, from $399 for the 42mm aluminum GPS model. It works only with an iPhone 11 or later on iOS 27 or later. Health features include a sleep score, sleep apnea and hypertension notifications and an ECG app, with no subscription listed. Apple rates the battery at up to 24 hours of normal use, so you'll charge it most days. The FDA-cleared hypertension feature flagged 41.2% of people with hypertension in its validation study, so getting no alert doesn't rule it out. Blood Oxygen and temperature sensing aren't for medical use.

Strengths on paper

  • From $399, Apple's price for the 42mm model, with no subscription listed for the health features
  • Sleep score, sleep apnea and hypertension notifications, an ECG app and irregular rhythm notifications on one watch
  • Fast charging: up to 80% in about 30 minutes, or up to 12 hours of normal use from 15 minutes
  • Water resistant to 50 m and IP6X dust resistant, with a water temperature sensor and a depth gauge to 6 m
  • A Ceramic Shield 2 front crystal on aluminum cases, which Apple says is 60% tougher than the Ion-X glass on Series 11

Limitations

  • Up to 24 hours of normal use (38 in Low Power Mode) means charging most days, and you'll need to fit charging around overnight sleep tracking
  • Works only with an iPhone 11 or later on iOS 27 or later
  • No power adapter listed in the box
  • Hypertension and sleep apnea notifications flag possible signs for follow-up rather than diagnose, and they're limited by age and prior diagnosis
  • Most people with hypertension got no alert in the validation study in FDA's clearance summary (the feature flagged 41.2%), so getting no notification doesn't rule it out
  • Apple says Blood Oxygen, Vitals and temperature sensing aren't for medical use
  • Apple says sleep apnea notifications also run on Series 9 and later, Ultra 2 and later, and SE 3, so they're not unique to Series 12

Energy component: Unknown

Purchase + installation + years × (annual consumables + annual other) + watts/1000 × hours/day × 365 × years × $/kWh. Annualized allowances, not an exact replacement schedule. Excludes taxes, shipping, finance and resale unless entered.
Replacement parts & cost assumptions

No scheduled consumable is listed. That does not mean zero maintenance or repair costs.

  • The estimate uses $399 as the purchase price, before tax. Apple's buy page lists Series 12 'From $399', and its lease example gives the 42mm model a $399 purchase price. With no required fee or part listed, the three-year estimate is $399 plus charging energy, which is unknown.
  • Other annual costs are $0 because Apple's Series 12 page lists no subscription for the sleep score, sleep apnea, hypertension, ECG or other health features. Optional paid services such as Apple Fitness+ and Apple Music aren't included.
  • Apple lists no replaceable parts or refills, so annual consumables are $0. That doesn't mean battery wear, replacement bands, repairs or AppleCare+ cost nothing.
  • Apple lists the watch, the band and a Magnetic Fast Charger to USB-C Cable in the box. It measured charge times with a 20W USB-C power adapter, which isn't listed in the box. Apple doesn't state the charger's power draw, so energy is unknown. With up to 24 hours of rated normal use, expect to charge most days.

  • Regulator recordFDA cleared Apple's Hypertension Notification Feature through 510(k) K250507 on September 11, 2025. It's Class II software for over-the-counter use by adults 22 and older who haven't been diagnosed with hypertension. Apple enrolled 2,229 adults without a hypertension diagnosis and asked them to wear Apple Watch for 30 days. An FDA-cleared home blood pressure monitor served as the reference. Among the 1,863 with at least 15 days of usable data, the feature flagged 41.2% of people with hypertension (sensitivity, 95% CI 37.2 to 45.3). It gave no notification to 92.3% of people without it (specificity, 95% CI 90.6 to 93.7).3 Hypertension was defined as an average systolic pressure of 130 mmHg or higher, or diastolic of 80 mmHg or higher. FDA's summary says the feature isn't intended for use during pregnancy. Nor is it intended to replace traditional methods of diagnosis or to monitor blood pressure. The summary also says the absence of a notification doesn't indicate the absence of hypertension. The clearance refers to compatible Apple Watch platforms without naming Series 12 or any other model.
  • Manufacturer claimApple says Series 12 has the most accurate heart rate sensing in a wearable. It says the watch takes background heart rate readings every five seconds and measures heart rate variability as often as every five minutes.1 Apple's footnote says the accuracy comparison comes from its own study against best-selling wearables in July and August 2026. This desk review found no independent Series 12 accuracy study.
  • Manufacturer claimApple's page says Series 12 has FDA-cleared features and sets intended-use limits on its health notifications. Sleep apnea notifications look for signs of moderate to severe sleep apnea in people 18 and older without a diagnosis. Hypertension notifications aren't for people under 22, people previously diagnosed with hypertension or pregnant people. The ECG app is for people 22 and older. Irregular rhythm notifications aren't for people under 22 or people previously diagnosed with AFib.1 This is Apple's own description of intended use. The Series 12 page doesn't list the specific FDA clearances.
  • Manufacturer claimApple says the Vitals and Blood Oxygen apps are for wellness purposes only and not for medical use. It says temperature sensing isn't intended for medical use, and that Cycle Tracking shouldn't be used for birth control or to diagnose a health condition.1 These readings describe trends. They aren't cleared diagnostic measurements.
  • Manufacturer claimApple's battery figures come from its own tests of preproduction Series 12 units in July and August 2026. For the GPS model, the all-day test covered 24 hours. It included 300 time checks, 90 notifications, 15 minutes of app use, a 60-minute workout with music playback and 6 hours of sleep tracking. The watch stayed connected to iPhone over Bluetooth throughout.2 Apple says battery life varies with use, configuration and many other factors, and that actual results will vary.
